Job Description :

This position works at both Geneva General Hospital and Soldiers & Sailors Memorial Hospital.

Education:
Minimum: Master's degree in Social Work (Bachelor's degree in Social Work with related work experience will be considered).

License/Certifications: LMSW or eligible is required. Must become licensed. Licensed Master Social Work (LMSW) is preferred.

Experience:
Minimum: New graduates considered.
Preferred:Previous related experience with Discharge Planning and/or Care Management. Experience in Acute Hospital setting and/or Nursing Home knowledge.

Other:
Minimum: Demonstrated ability to handle confidential information with discretion and ability to deal with the people in a professional and courteous manner. Ability to meet deadlines, manage multiple priorities and enhance the spirit of teamwork through effective role modeling. Excellent interpersonal, communication and organization skills. Computer literacy.
Preferred: Experience with Microsoft Office products and electronic medical record.
