Nurse Practitioner Program Highlights:

Nurse Practitioner Duties and responsibilities:

  • Conduct Health Risk Assessments (HRA) on insured plan patients virtually.
  • Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit
  • Conduct necessary screenings to capture HEDIS measures and close gaps in care
  • Educate and counsel patient and family on any conditions identified during assessment

Candidate Requirements:

  • Must have clear, active Nurse Practitioner state license and board certifications
  • Must have at least 1 year experience with Geriatric population and/ or home health
  • Must be enrolled in PECOS
  • HCC experience/ knowledge preferred
  • Ability to complete and clear a background and credentialing screen
  • Strong computer skills: including but not limited to Word, Excel, EMRs and Web based applications
  • Must be able to complete all HRAs and corrections (if needed) in a timely manner

Job Type: Part-time

Pay: $70.00 - $490.00 per day
