UW Medical Center – Montlake Heart Institute has an outstanding opportunity for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Cardiac Transplant team. The APPs ensure that heart transplant recipients are treated with appropriate immunosuppressive and anti-infective therapies, do not have signs and symptoms of infection, rejection, and are optimally medically managed to prevent complications and hospitalizations. The APPs optimize goal directed medical heart failure therapies in patients with AHF including evaluating and treating patients with acute decompensated heart failure, clinically relevant atrial or ventricular arrhythmias and maintenance of chronic advanced medical heart failure therapies.

POSITION DETAILS
Full-time (100% FTE), Outpatient Ambulatory APP, Day Shift 40 hrs/week

RESPONSIBILITIES
  • Collaborate with an inter-professional team including APPs, cardiologists, nurses, medical assistants (MAs) and clinic schedulers to provide high quality and safe, patient- and family-centered care in a premier institution

  • Independently and collaboratively provide comprehensive care to 4-6 advanced heart failure or heart transplant patients in each of eight 4-hour blocks weekly (based on 1.0 FTE) in the outpatient setting

  • Diagnose, treat and manage acute and chronic heart failure, heart transplant, and heart failure-related illness

  • Order and interpret pertinent laboratory and diagnostic imaging in a timely manner

  • Prescribe and titrate diagnostic, pharmacologic and therapeutic interventions, including immunosuppressive medications

  • Communicate in a professional, timely and respectful manner with all members of the health care team whether through verbal, email or secure text communication

  • Educate team members including physicians, nurses, MAs, patients and families on AHF therapy and heart transplant needs and management

  • Document per Heart Institute Standards via EPIC

  • Participate in quality improvement projects and process improvement activities

  • Participate in and support the Division of Cardiology’s education, research and quality initiatives

  • Provide education and mentorship to fellow team members, staff, nurses and students alike

  • Attend weekly Cardiology Grand Rounds, clinical case conferences and quarterly faculty meetings

  • Follow University of Washington Medical Center (UWMC) Professional Behavior Policy and Procedures


  • Required Qualifications:
  • Graduate Degree from and accredited Nurse Practitioner or Physician Assistant Program, or foreign equivalent

  • Current and valid state Registered Nurse and Advanced Practice Nursing Licensure for ARNP or Physician Assistant licensure for PA-C

  • Nationally certified ARNP/PA with DEA

  • Highly reliable, flexible provider with proven excellence working in an interdisciplinary collaborative team environment

  • Effective interpersonal skills, leadership skills, and clinical expertise in the care of cardiology patients

  • Able to communicate effectively in English, both verbal and written

  • Basic computer knowledge and command of basic personal computer skills


  • Ideal Qualifications:
  • Training and experience in cardiology including heart failure, cardiac transplant

  • ACLS Certification

  • Well versed in EPIC
