Family Health Services is a Federally Qualified Health Center (FQHC) that provides quality, affordable primary care, dental, and behavioral health services. Our “high touch, high tech” clinics offer unique patient focused care to anyone in the community, regardless of their ability to pay. We are rapidly growing in Sandusky, Ohio at our two locations as well as in Norwalk, Ohio and surrounding areas. We also offer behavioral health and primary care at several local schools in hopes of eliminating barriers to those needing services.
Family Health Services is looking for a provider that embodies our core values of innovation, collaboration, adaptability, and integrity to ensure we achieve patient centered excellence. We are recruiting a team-oriented Social Worker or Counselor that would be located in one of our offices or in a school-based setting. This person would complete intake assessments, provide individual/family therapy, ensures documentation is completed within specified timeframe in our electronic medical records, coordinate between other departments and agencies, assist in general case management, possess problem-solving skills, and be able to enjoy a fast-paced environment.
This position is full-time, Monday through Friday - no weekends, holidays, or call required!
Requirements include a Bachelor’s degree in Social Work or Counseling with a valid professional license such as LSW or LPC. Preferred Master’s degree with license such as LSW, LISW, LISW-S, LPC, LPCC, LPCC-S. Preferred experience working with children and adults in an office, school, or community setting.
Perks we offer Social Workers needing to obtain 150 hours of supervision post their Master’s in order to achieve independent licensure, will be offered individual and group supervision for free during scheduled work hours. Family Health Services also offers tuition reimbursement and funds for continuing education.
Loan forgiveness option for full time LISW and LPCC as Family Health Services is a National Health Service Corps (NHSC) approved health care site. Able to apply for the NHSC Loan Repayment Program with a 2-year full time service commitment if awarded.
