We are currently seeking a social worker to join our team Monday-Friday, 8a-5p, covering our South Denver Metro area of service. Hospice experience is a plus--we will also consider similar experience for someone wishing to enter the field of hospice medicine.

The social worker meets families at a sensitive time: making the end-of-life journey. In this role, you are a voice of understanding, serving as advocate, making assessments and communicating with the interdisciplinary care team (IDT) as needs and conditions change.

Skills and Attributes:

• Helping the members of the IDT understand the psychosocial, emotional, and financial factors related to the patient and family during and after the terminal phase of illness (including the effects of caregiving).
• Educating patients/family about decision-making and advance directives, providing materials and assisting with completion as necessary.
• Offering short-term individual counseling and assisting in crisis intervention.
• Helping to indentify patient/family strengths and support systems available to ensure comfort, dignity and quality of life.
• Understanding the Medicare Hospice Benefit, Medicaid and private insurance benefits and options for the uninsured.
• Helping with patient referrals/admissions as requested.
• Coordinating emergency, temporary, or permanent placement and completing PASRR as needed.
• Maintaining proper and timely records of visits, contacts, assessments, and interventions with patients and families.
• Identifing and utilizing community resources and assisting patient/family to access them.

Required:

• Masters degree in Social Work.
• Two years' experience working in hospice care or a related field.
• Valid Colorado driver's license, proof of auto insurance and reliable transportation.
