Fedelta Home Care is a leading in-home healthcare agency that has an exciting opportunity for a pediatric/adult Registered Nurse Case Manager to join our team! In this role you will be responsible for working with vulnerable clients, adult and pediatric, acting as both a guide and advocate. You will assist clients in attaining their maximum functional potential, encourage independence and keep them at home with their families and loved ones while addressing safety, security, and the health of clients.


Skills, Certifications, and Requirements:

  • Current RN License in good standing and valid in Washington State required;
  • Current and up-to-date BLS card required;
  • 3-5 years experience in RN case management preferred;
  • 1+ years' experience working with patients on ventilators and with tracheostomy required;
  • Covid, TB, and Flu vaccinations are required;
  • Valid/current driver's license and auto insurance required;
  • Satisfactorily pass criminal history and reference checks;
  • Must have excellent interpersonal and communication skills;
  • Good organizational skills;
  • Ability to work independently and cooperatively with others;
  • Ability to maintain confidentiality;
  • Ability to be flexible and perform work under time pressure;
  • Ability to work with people of diversity;
  • Proficient in MS Office Suite, e.g. PowerPoint, Excel, Word, etc.;

Salary:

  • $90,000-$120,000 DOE

Essential Duties and Responsibilities:

  • Apply nursing assessment skills, judgment, and effective communication skills to open new and existing case panels of Home Health cases with the goal of creating a safe and therapeutic residential environment while maintaining compliance with Washington State Home Health regulations (WAC 246-335), and Fedelta standards, policies, and procedures. Promote client safety and quality of care in their home and community beginning on the day of admission.
  • Compile and implement individualized Home Health Plans of Care (POCs) as assigned based on physician orders, nursing needs assessment, and client input. Create client's initial chart forms and evaluate the effectiveness of POCs on an ongoing basis. Work with clients and their prescribing providers to revise as necessary based on appropriate goals and evaluation outcomes. Promptly process and communicate changes in the POC to the client, family, and appropriate nursing staff.
  • Evaluate the safety of the client's home environment and recommend changes as needed.
  • Ensure the client's physicians and other appropriate health care providers are advised of the client's response to care and any significant change in the client's condition or needs, as necessary.
  • Conduct a thorough Beside Orientation (BSO) for each new attending nurse that has been assigned to a client on the Nurse Care Manager's caseload.
  • Conduct visits with each attending nurse assigned to the Nurse Care Manager's caseload a minimum of once per month. Address questions or concerns related to ongoing patient care, interpersonal dynamics in the home environment, scheduling, or aspects related to employment with Fedelta. Forward any concerns not directly related to patient care or interpersonal dynamics of the home to the appropriate administrative team members.
  • Perform supervisory visits for each in-home client on the Nurse Care Manager's caseload at a minimum of every 60 days.
  • Provide patient education and resources to clients, family members, informal caregivers, and related staff that promotes safe and appropriate care.
  • Provide direct care to clients as an emergency measure to protect the safety of the client and maintain an acceptable level of client satisfaction when necessary.
  • Accurately and concisely document pertinent information in the client's clinical record including but not limited to; chronological care management notes, care provided, client's response to care, changes in condition and needs, progress towards established goals, and factors that may impact the quality of services or client satisfaction.
  • Perform monthly chart audits to ensure timely and accurate implementation of prescriber's medical orders and professional documentation of care.
  • Familiarize yourself with all Fedelta home health client cases and plans of care to provide direct patient care or Bedside Orientation (BSO) for the benefit of a case for other Nurse Care Managers assigned caseloads as needed.
  • Assist the Director of Clinical Services with annual skills assessments, performance evaluations, and coaching of licensed nurses as directed.
  • Contribute and assist with the training of Fedelta Home Care staff regarding home health care and other related topics.
  • Assist the Director of Clinical Services in ensuring compliance with Washington State Home Health regulations (WAC 246-335).
  • Perform case management-related errands such as delivering physician orders, replenishing blank chart forms to client homes, transporting borrowed ventilators used for training purposes, etc.
  • Conduct yourself professionally and knowledgeably while representing Fedelta Home Care to clients.
  • Collaboratively work with Home Care services and Client Care Managers to provide exceptional care results for clients.
  • Maintain strict confidentiality with sensitive information and proprietary company information. Adhere to HIPAA privacy practices and confidentiality as required by regulation.
  • Maintain a high level of professionalism in conduct and communication with team members, clients, families, and vendors while in engaged in work responsibilities for Fedelta Home Care.
