At least a bachelors degree in social work from a school of social work accredited by the Council on Social Work Education. This position is paid per visit.

Demonstrates knowledge and use of clinic policies and procedures as well as applicable federal and state rules and regulations.

If licensed in Indiana must have a controlled substances registration certification and comfortable in writing class II to V controlled substances required.

Must have and maintain a valid driver's license and acceptable driving record in accordance with Mayo Clinic Policy. Current ITLS/ATLS or PHTLS.
