The Pavilion at Ocean Point is a 133-bed Skilled Nursing facility located in the Point Loma area of San Diego. We have an immediate opening for a full-time Licensed Phyical Therapist.

Duties include:

• Evaluate patients and develop effective treatment plans
• Obtain approval for services from referring physician and treat patients according to prescribed treatment plan.
• Support nursing staff with training of Restorative Aides.
• Direct Physical Therapy Assistants in direct patient care and associated activities.
• Communicate with appropriate staff and other parties regarding patient progress and challenges.
• Participate in conferences and meetings as needed.
• Contribute to training programs for other staff in the facility.
• Evaluation and record daily treatment notes and changes, weekly progress notes, and recertification documentation, per all appropriate requirements.
• Prepare for discharge by educating patients, their families, and nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Recommend and facilitate the ordering of medical devices for patients to facilitate independence in mobility (walkers, crutches, braces, canes, etc.).
• Maintain positive, effective, and appropriate relationships with patients, their families, and staff.
