Our outpatient hand therapy practice is looking for a compassionate, forward-thinking licensed and / or Certified Hand Therapist or Occupational Therapist with hand therapy experience. Certified Hand Therapists would be a welcoming addition to the team as well.
FORM Hand Therapy is part of a Hand, Wrist & Elbow Institute in North Silicon Valley. Most patients are referred by an in-house hand surgeon who performs cases at an outpatient surgery center located in the same facility in addition to other outside providers.
The therapist’s scope of work will focus on the hand and will include the identification, prevention, remediation, and rehabilitation of patients who have impairments, functional limitations, disabilities or changes in physical function, activities of daily living and health status resulting from injury, diseases or other causes.

Responsibilities and Duties
- Integrate the elements of patient management - evaluation, examination, diagnosis, prognosis, and intervention - in a manner designed to maximize outcomes taking into consideration the patient's goals.
- Custom splint fabrication and post operative therapy are an integral component of the department.
- The therapist will interact and practice in collaboration with surgeons, primary care providers, therapists and assistants.
- The therapist participates in carrying out the objectives of this service by performing duties which are commensurate with his / her education
- Evaluate and report on patient's progress.
Qualifications and Skills
- Valid State Occupational Therapist License
- Excellent work ethic and dedication to patient success
- CPR and first aid certification required
- Individual must be a team player, deliver exceptional quality care, able to multi-task and work in a fast pace environment.
- Bilingual skills preferred but not required
