Essential Functions

The Home Health Aide is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional wellbeing, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures.

• Understand and adhere to established agency policies and procedures
• Perform personal hygiene and hands-on care as assigned
• Complete appropriate visit records in a timely manner as per agency policy
• Reports changes in the patient’s condition and needs to the RN
• Performs household services essential to health care in the home environment
• Ambulates and exercises the patient as needed
• Perform simple procedures as an extension of the therapy or nursing services
• Assists with medications that are ordinarily self-administered as needed
• Attends in-service and continuing education programs as scheduled as necessary

Job Qualifications and Requirements

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency
• Have a sympathetic attitude toward the care of the sick and elderly
• Ability to carry out directions with exceptional communications skills
• The maturity and ability to deal effectively with the demands of a home care position

License Requirements

• Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
• CNA (Preferred)
