Current Arkansas license as a Physical Therapist. KULA has many licensed occupational therapists, physical therapists, and speech-language pathologists on staff

Must possess Virginia DEA license; Licensed as a NP or PA in the state of Virginia; Promote teamwork between clinical and administrative staff;

Escalates calls to supervisor when necessary and appropriate. Responds to requests for assistance and/or possible processing of technical support as needed.

Current license or eligibility for license to practice physical therapy by the Colorado Department of Regulatory Affairs (DORA).

Must be licensed and insured with a clean driving record and own a reliable vehicle (vehicle can be provided in very exceptional circumstances).
